Yeah, both times my truck did that I was needing injectors and replaced them and it went away. I was kinda hoping that would not be the case with yours. If you do get new injectors make sure you oil sample. My new ones leaked right away and I ended up having 10% fuel dilution.

Usually when this happens its an issue with the ECM. If you want find a Detroit shop and see if they will hang a test ECM on it for you.

start with the basics check your batteries cables (both positive and grounds) allconnections at the starter and alternator and frame and engine block also check the ecm power harness the S60 dde has a 3 wire harness ( 2 pos 1 neg) and if a positive goes bad it will go so far as to cut three cylinders out (same as half engine idle mode)

my laptop was down, sorry.every thing is good and tight on ecm, starter and alternator.ive just been running it.you just cant take off fast, easy on throttle in the begining,if to much pedal it will hasatate with smoke and then clear right up and run good

i had a similar issue. i would describe my problem as rough idle. i i did many things (changed fuel lines, new injectors as a a part of an inframe, installed a fass, test cac, recent overhead, oil sample, etc.). i never completely did not ever have a periodic rough idle until i went to detroit shop to turn up ecm. they found ecm was programed for a different engine model. the ecm was set for a pk60 but engine is a gk60 model. it took them less than 1 hour to reprogram and engine hardly ever hiccups or idles rough. however, if you are going to mess with ecm, i would stay away from detroit to "tune up" or "flash". my engine's response to the throttle is slower-i suppose for emmissions.

I am having they same issue. Mine really misses bad. Sounds like it is only on 3 cylinders. Really a pain when loaded heavy, trying to pull out. Once it hits 1000 rpm clears up and runs fine. Getting good fuel mileage hate to mess with it. Tried another ECM, same issues. Installed FASS and all new fuel lines, no changes. My mechanic plugged it in and checked injectors they all tested fine. He told me to keep running it.

If it is an injector issue and when you replace injectors, I recommend installing a FASS that would filter the fuel down to 3 microns and take the air out of system to make those injectors last alot longer.
